Clostridium botulinum

Clostridium botulinum is a gram-positive, spore-forming anaerobic bacterium that produces botulinum toxin, the cause of botulism. The toxin blocks acetylcholine release at the neuromuscular junction, producing a descending flaccid paralysis.

The toxin is a protease that cleaves SNARE proteins in motor neuron terminals, preventing the release of acetylcholine at the neuromuscular junction. Without acetylcholine signaling, muscles cannot contract, producing a symmetric, descending flaccid paralysis. Classic early findings are the "4 Ds" — diplopia, dysarthria, dysphagia, and dyspnea — with progression to respiratory failure in severe cases. Unlike tetanus toxin, which blocks inhibitory neurotransmitters and causes spastic paralysis, botulinum toxin silences excitatory signaling and causes floppy paralysis.

The three main forms of botulism reflect how the toxin is acquired. Foodborne botulism follows ingestion of preformed toxin, classically from improperly home-canned foods. Infant botulism occurs when ingested spores — famously from honey — germinate in an infant's immature gut and produce toxin, causing the "floppy baby" syndrome; this is why honey is avoided before age one. Wound botulism arises when spores contaminate a wound, often in injection drug users. Treatment centers on antitoxin and supportive care, including ventilation. In controlled doses, the same toxin is used therapeutically and cosmetically as Botox.
