Metabolic syndrome

Metabolic syndrome is a cluster of conditions — abdominal obesity, high blood pressure, elevated fasting glucose, high triglycerides, and low HDL cholesterol — that together sharply raise the risk of type 2 diabetes and cardiovascular disease.

Metabolic syndrome, historically called syndrome X, is not a single disease but a cluster of interrelated metabolic abnormalities. The five components are abdominal (central) obesity, elevated blood pressure, elevated fasting glucose, elevated triglycerides, and reduced HDL cholesterol. A diagnosis generally requires at least three of the five, with each component defined by standard clinical cutoffs.

The unifying mechanism is insulin resistance. Excess visceral fat releases free fatty acids and inflammatory signals that blunt tissue response to insulin. The pancreas compensates with hyperinsulinemia, but glucose still trends upward; the liver overproduces triglyceride-rich particles while HDL falls; and insulin resistance plus sympathetic activation raise blood pressure. Each abnormality reinforces the others, which is why the components so often travel together.

The syndrome matters because of what it predicts: substantially elevated risk of type 2 diabetes mellitus, atherosclerotic cardiovascular disease, and nonalcoholic fatty liver disease, which can progress toward cirrhosis. Management targets the root causes — weight reduction, physical activity, and dietary change — alongside treatment of the individual components.

Metabolic syndrome is tested across the health professions. The USMLE Step 1 covers its diagnostic components, insulin-resistance pathophysiology, and hepatic complications, while the NPTE and NPTE-PTA exams test it within endocrine and metabolic systems, including its implications for exercise prescription and rehabilitation in affected patients.
