Taenia

Also known as: tapeworm, taeniasis

Taenia is a genus of parasitic tapeworm that infects humans through undercooked meat. Taenia solium comes from pork and Taenia saginata from beef, and only T. solium can also cause cysticercosis.

Taenia species are cestodes — flat, segmented intestinal worms that attach to the small bowel with a head called a scolex and grow a chain of segments known as proglottids. Humans are the definitive host for both medically important species, harboring the adult worm and shedding eggs and gravid proglottids in stool.

The two species are acquired the same way but differ in what follows. Eating undercooked beef containing larval cysts transmits Taenia saginata, which causes intestinal taeniasis and nothing more; its scolex bears four suckers and no hooks. Eating undercooked pork transmits Taenia solium, whose scolex carries four suckers plus a hooked rostellum. Intestinal infection with either is often mild, producing vague abdominal discomfort or the alarming but harmless observation of proglottids passed in stool.

The critical distinction is what happens when a person swallows T. solium eggs rather than larval cysts in meat. Eggs are shed by a human carrier and transmitted by the fecal-oral route, so infection can occur without eating pork at all. Larvae hatch, cross the intestinal wall, and encyst in tissue as cysticercosis. When cysts lodge in the central nervous system the result is neurocysticercosis, which typically presents with new-onset seizures and shows multiple cystic brain lesions on imaging; in endemic regions it is a leading cause of adult-onset epilepsy. T. saginata eggs do not produce this syndrome.

Diagnosis of intestinal infection rests on identifying eggs or proglottids in stool, though Taenia eggs of the two species are morphologically identical; species identification relies on proglottid uterine branching or on molecular testing. Standard management uses praziquantel for the intestinal worm, while albendazole is the agent used for tissue cysticercosis. A useful contrast on exams is Diphyllobothrium latum, the fish tapeworm, which causes vitamin B12 deficiency — an effect Taenia does not produce.

USMLE Step 1 parasitology questions on Taenia almost always hinge on the pork-versus-beef source and on recognizing that ingesting T. solium eggs, not larvae, causes cysticercosis. A vignette describing new seizures with cystic brain lesions in a patient from an endemic area is the canonical presentation.
