Venipuncture

Venipuncture is the procedure of puncturing a vein with a needle to collect a blood specimen or gain venous access. It is the most common method of blood collection, typically performed on veins in the antecubital area of the arm.

Venipuncture is the puncture of a vein — most often to draw blood for laboratory testing, and also to start IV therapy or administer medications. Routine draws use the veins of the antecubital fossa (the inner elbow): the median cubital vein is the first choice because it is large, stable, and well anchored, with the cephalic and basilic veins as alternatives. The basilic vein is the last resort of the three because it lies near the brachial artery and a major nerve.

A routine venipuncture follows a standard sequence: verify the order and the patient's identity, assemble supplies, apply a tourniquet no longer than about one minute, select and palpate the vein, cleanse the site with antiseptic and let it dry, anchor the vein, insert the needle bevel-up at roughly a 15–30 degree angle, fill tubes in the correct order of draw to prevent additive cross-contamination, release the tourniquet, remove the needle, activate the safety device, and apply pressure to the site. Proper labeling at the bedside is essential to specimen integrity.

Technique matters because complications are common and mostly preventable: hematoma from through-and-through puncture or inadequate pressure, hemolysis from a too-small needle or vigorous handling, nerve injury from poor site selection, syncope, and collapsed veins. Prolonged tourniquet time causes hemoconcentration that skews results.

Venipuncture is a core competency on the CCMA (Certified Clinical Medical Assistant) exam, which tests the routine procedure step by step — site selection, order of draw, tourniquet limits — along with recognizing and preventing the problems associated with blood collection.
